Welcome to Broadway Family Medicine! As family physicians, we endeavour to provide thorough, compassionate and state of the art medical care in a friendly and welcoming environment.  Please read the following information sheet – it explains important details about how our office works.  

 

FAMILY HEALTH ORGANIZATION (FHO)

 

FHOs are primary care models that focus on complete medical care, emphasize preventative health measures, and provide patients with 24-hour access to health care. The Yongeline FHO is comprised of 20+ Family Physicians working out of 3 practices in our building at 2401 Yonge Street, and 1 practice at 1881 Yonge Street (Yonge and Davisville). To join our FHO, you will complete a form at your first visit that will enroll you on your doctor’s roster and ensure you have access to our After Hours Clinic.

CLINIC POLICIES

 

TRUST AND RESPECT

Of great importance to the successful operation of this clinic is mutual trust and respect between patients, doctors and staff.  You can expect us to treat you with respect, and in return we expect the same of our patients.  Inappropriate behaviour in the office will not be tolerated.  

 

CONTACTING THE OFFICE

Telephone access to the office can at times be associated with some delays.  Please be patient.  As another option, you can also request an appointment via e-mail at:

hEALTH cARDS

Please bring your health card to each visit.  If your health card has expired, please renew it before your appointment.  Patients without a valid health card are required to pay for the visit on the day of the appointment.

 

SCHEDULING  

We try our best to run on time out of respect for your busy schedule.  However, it is not always possible to predict how much time a given patient will require, and extenuating circumstances will arise.  We appreciate your patience if we are ever running late, and promise to provide you with the time you are entitled to.  To assist us in running on time, we ask that you please do not come late to your appointments.  If you are running late, please notify us.  Patients arriving late may be asked to reschedule their appointment for another day. If you need to cancel an appointment, please give us 24 hours notice so that your appointment can be given to another patient.  There are fees for missed appointments, and patients with repeated missed appointments may be asked to leave the practice.

URGENT CARE

If you need to see your doctor urgently, please contact the office early in the day to let us know.  We set aside appointments for urgent issues, and ask that you please do not seek care elsewhere.  PLEASE DO NOT USE WALK-IN CLINICS or HOUSE-CALL SERVICES whenever possible.  These services compromise the quality and continuity of your medical care.  In addition, your doctor pays personally for the costs of those visits.  Frequent visits to walk-in clinics may result in you being de-rostered from our active patient list.

Tell our staff if you have an urgent problem.  We endeavour to see patients with urgent health problems with same day appointments.  If your doctor is not available, you may be able to see one of the other doctors at our clinic.  If that is not possible, you can see one of the doctors with the Yongeline FHO’s After Hours Clinic.  Please see the below section on After Hours Coverage for more details on what you can do on the rare occasion that we are unable to accommodate you.

 

aFTER hOURS cOVERAGE

For information regarding After Hours Coverage please call the Yongeline After Hours Information Line at (416)231-7226.  After Hours Clinics are available through the FHO Monday-Thursday from 5:00-8:00pm and on one day of the weekend, generally Saturday from 9:00am-12:00pm (please confirm the weekend clinic time by calling the information line as days and times may vary).  Call the information line after 4pm to find out which doctor is on call.  You can then call that doctor’s office to book an appointment for that day.   

Outside of these times, After Hours services are available to you through the Telephone Health Advisory Service which can be reached 24 hours a day by phoning     1(866)553-7205.

 

pRESCRIPTIONS

We will try to give you enough medication until your next appointment is due.  If you are running low on a prescription medication, it probably means it is time for you to see your doctor again.  Please ensure that you have a follow-up appointment before your medication runs out.  Prescription renewals by telephone or fax involve a great deal of time.  It is not a service covered by OHIP, and a charge will apply.  

eLECTRONIC mEDICAL rECORDS & eMAIL

Our clinic uses electronic medical records to provide you with the safest and most efficient way of compiling and maintaining your medical chart.  

You can communicate with our office via e-mail to schedule appointments, inquire on the status of a referral, or make simple requests.  You may also ask medical questions, and the staff will pass on your question to your doctor.  Patient requests for e-mail medical advice are an uninsured service and a charge will apply. This service is included in the annual block fee.  Please note that it may take two business days for staff to respond to e-mails, and we do not respond to e-mails on evenings and weekends. If you have an urgent matter, please call the office directly.  Please ensure you have read our Physician-Patient E-mail Communication Policy and signed the Consent Form. This can be done in person or online at our website. You can also see our office website for information regarding our E-mail and Privacy Policies.

 

UNINSURED SERVICES

There are a number of services that are not covered by OHIP, including doctor’s notes, notes for massage therapy, prescription refills by phone/fax, form completion and examinations requested by a third party. These are listed in a separate letter found within this package, and on our website. You have the option to take part in the annual fee program as detailed in that letter or you can choose to pay for services when they are provided.

 

MEDICAL STUDENTS & RESIDENTS

Broadway Family Medicine is proud to be a teaching clinic affiliated with the University of Toronto. From time to time, you may encounter student doctors at various levels of training. As patients, you will play a vital role in their education. At all times, your doctor will remain completely responsible for your care and will directly supervise these students. We hope you will feel comfortable having students involved in your care. If you have any questions or concerns regarding medical students, please feel free to raise them with your doctor.

PREVENTATIVE MEDICINE

 

We would like to take this opportunity to advise you about some medical tests, investigations and immunizations which you should consider.

 

IMMUNIZATIONS

  • Childhood Immunizations – Will be discussed on an individual basis

  • Hepatitis A&B Immunization - Recommended for travel to most international destinations

  • HPV Immunization – Indicated for males aged 9-26 and females aged 9-45

  • Pneumococcal Immunization - Over 65 years old or with chronic illness

  • Seasonal flu vaccine - All patients over 6 months old (especially infants & children, pregnant patients, caregivers of young children, patients over 65 years old and patients with chronic illness), every year

  • Shingles (Herpes Zoster) Immunization - Patients over 60 years old

  • Tetanus Immunization – Adults, every 10 years

 

SCREENING TESTS  

 

FEMALES

  • Mammogram - Age 50-74 - every 1-2 years

  • Pap test - Age 21-70 - every 3 years

 

MALES  

  • Prostate Cancer Screening - Please discuss this with your doctor.

 

ALL PATIENTS

  • Fecal Occult Blood Test – Age 50-74 - every 2 years (Testing stool for microscopic blood to detect early stages of bowel cancer); collection kits are available in the office
