Our office uses Electronic Medical Records to assist us in providing you state of the art medical care. We are located at the heart of Toronto on Yonge Street just north of Eglinton. We are close to many amenities, and our medical building is equipped with a lab, diagnostic imaging, and a pharmacy.
We focus on preventative, comprehensive and continuous health care, and our physicians are well-equipped to meet your medical needs.
Dr. Valerie Ha graduated from the University of Toronto's Medical School in 2000 and completed her Residency at the University Health Network.
She has worked in primary care at a community health centre and in acute care as a hospitalist.
She will be joining the practice as of Sept. 6th 2016 and is looking forward to being part of the team.

Dr. Jordana Kline
Dr. Jordana Kline was born and raised in Toronto. She completed her undergraduate studies in Psychology at York University, and then attended medical school at McMaster University.
She returned to Toronto for her residency in Family Medicine, which she completed at Toronto East General Hospital through the University of Toronto.
Dr. Kline enjoys providing a comprehensive and continuous health care to people of all ages.
Dr. Marla Munk was born and raised in Toronto. She completed her undergraduate studies in Psychology at the University of Western Ontario, and obtained her medical degree at the University of Toronto.
She continued at the University of Toronto to complete her residency in Family Medicine at the Toronto Western Hospital. She is a lecturer in the Department of Family and Community Medicine at the University of Toronto and helps to train medical students.
Dr. Munk has been practicing comprehensive, patient focused care since 2003. She enjoys caring for patients throughout their lifespan, from newborns to senior citizens.

Dr. Steven Rothman was born and raised in Montreal. He completed a bachelor of science degree in Psychology at McGill university, a 2 year masters program in Nutritional Science at University of Toronto and then returned to McGill to complete his medical degree. He then completed his Family Practice residency and served as Chief Resident at Mt. Sinai Hospital through the University of Toronto.
Dr. Rothman has been providing comprehensive primary care to children and adults since 1995. He mentors medical students and is also the lead physician of our Family Health Organization, Yongeline FHO.

Dr. Carolyn Shedletzky was born and raised in Toronto. She completed her undergraduate studies in Psychology at the University of Western Ontario, and then attended medical school at McMaster University.
She returned to Toronto for her residency in Family Medicine, which she completed at North York General Hospital through the University of Toronto in 2010. Dr. Shedletzky enjoys practicing the full scope of Family Medicine, including providing care to people of all ages, from newborns to seniors. She is focused on providing comprehensive and patient-centered primary care.

The Yongline Family Health Organization (FHO) is comprised of 20+ Family Physicians working out of three practices in our building at 2401 Yonge Street, and one practice at 1881 Yonge Street (Yonge and Davisville).
FHOs are primary care models that focus on complete medical care, emphasize preventative health measures, and provide patients with 24-hour access to health care. By joining our FHO, you will have the benefit of accessing our After-hours Clinic.
